Assessment is made independently, not by the prescribing physician, but the responsible agency as a rule consults him for his observations and prognosis. The individual assessment of the patient's driving ability does not use uniform criteria. As a rule, a minimal duration of 6 to 12 months on substitution with stable dosage is required. Other requirements are an adequate status in somatic and psychological health, a stable housing condition, absence of delinquent and otherwise irresponsible behavior, and abstinence from illegal and non-prescribed substances (documented through urine analysis).
Initial doses correspond to the extent of opiate tolerance in a given patient. They are to be handled cautiously in order to avoid overdose. If tolerance is not clearly evident, doses should not exceed 30mg/day, in cases of evident tolerance 60mg/day split in two partial doses should not be exceeded. Increasing of doses is following observations on withdrawal symptoms. It is advisable to see the patient once or twice a day in order to prevent unpredictable complications. Only exceptionally it is necessary to hospitalise a patient for determining the adequate dosage.
